Cygnet Primary Care Network (PCN) comprises five GP practices located in the East Riding of Yorkshire:

  • Bartholomew Medical Group
  • Gilberdyke Health Centre
  • Howden Medical Practice
  • Montague Medical Group
  • The Snaith & Rawcliffe Medical Group

Together, we form a part of the NHS Long Term Plan. Our network covers over 50,000 patients

As a new and developing network of GP Practices, we work at scale to develop local health services and aim to more effectively meet the needs of all our patients. Our team is comprised of many disciplines including a wide range of highly-skilled local health professionals, management experts, and dedicated support staff.

Through experience, existing data, new research and talking with patients, we will identify the current and future needs of our population. We will work at scale and in partnership with acute and community trusts, mental health, social care and voluntary sectors. Applying innovations, our plans will aim to deliver improvements in services, including faster diagnosis, better treatments, and improved prevention.
