PCNs are set-up to operate with competent leadership and an accountable workforce where each person understands their role and responsibilities. In the coming months, Cygnet PCN will be recruiting multi-disciplinary team to help us manage, coordinate and deliver relevant services to patients.

PCN Director

Dr Martin is a General Practitioner at the Snaith & Rawcliffe Medical Group. 

His special interests are mental health, pain management and hypertension.

The surgery attained the Team Excellence Award from the Hull York Medical School.

Nicola Price

Clinical Pharmacy Technician

Nicola is a Clinical Pharmacy Technician working over all five practices in Cygnet PCN.

Her extensive experience in the field includes fourteen years work in community pharmacy.

Prior to joining the PCN, Nicola spent three years working closely with numerous residential and nursing homes in York and the surrounding area.

This included the provision of training , medication and support to residents and the care home teams

She is passionate about providing excellent quality care to the most vulnerable members of our society. 


 

Gemma Oughtred

Social Prescriber

Gemma has joined Cygnet PCN as a Social Prescriber. She is a member of the NALW (National Association of Link Workers). Social prescribing is designed to support people with a wide range of social, emotional or practical needs.

Many schemes are focused on improving mental health and physical well-being. Those who could benefit from social prescribing schemes include people with mild or long-term mental health problems, vulnerable groups, people who are socially isolated, and those who frequently attend either primary or secondary health care.

Gemma is currently enrolled on a financial welfare course. This will enable her to provide an even wider range of support to her clients. 


Sam Swallow

Paramedic

Sam has joined Cygnet Primary Care Network as a paramedic working across all five of our practices.

He qualified with a BSc in Paramedic Practice at Teesside University and went on to gain three years experience with Yorkshire Ambulance Service in Urgent & Emergency Care.

He is passionate about patient-centred care and making every patient contact count.

In his spare time, he participates in motor sport and plays musical instruments - including guitar and piano.

He also enjoys travelling and experiencing new cultures. 


Isma Butool

Paramedic

Isma has joined Cygnet PCN as a paramedic

She graduated with a BSC (Hons) in Biomedical Science from Manchester University in 2011. She then qualified as a Paramedic after studying Paramedic Practice at Sheffield University in 2018. As a qualified paramedic, she gained 2 and half years experience working for Yorkshire Ambulance Service (YAS).

Whilst focusing generally on the clinical needs of care home patients, Isma will work to support priority patient care across all of the Cygnet practices.

Her hobbies and interests include socialising, travelling and keeping fit.


Sharon Burgin

Clinical Pharmacy Technician

Sharon is a clinical pharmacy technician working over all 5 practices for Cygnet PCN.

She has worked in pharmacy for over 18 years and has experience on hospital wards and in outpatients. As an accredited checking technician she has also worked in the community.

She is passionate about making sure everyone, young and old, gets the best care possible. Her hobbies include dog walking, watching football and rugby league and swimming.


 

Caroline Bell

Care Co-ordinator

Caroline Bell has been appointed as a Care Coordinator with Cygnet Primary Care Network.

She has ten years experience of working in older peoples specialist mental health services.

Prior to that, she worked as an administrator in the both the education and retail sectors.

Caroline's spare-time interests include dog walking and reading. She also enjoys arts and crafts.


Olivia Whitehead

Olivia Whitehead

Care Co-ordinator

Olivia has been appointed as a Care Coordinator at Cygnet Primary Care Network.

Prior to this, she worked in a doctor's surgery, dentistry and child care.

The care coordinator's role is to consult with patients and their primary care providers, explaining their conditions and answering their questions, helping oversee their treatment plans and monitoring their progress.

Olivias hobbies are horse riding, socialising, and travelling.
